If you change insurance companies or join another company’s plan, please be sure to let us know. If your insurance requires a referral, please bring the referral with you to your appointment.

Please note: If your plan is listed on the Washington Healthplan Finder, we recommend contacting our office to confirm that your specific plan is in network with our providers.

We will bill your insurance as applicable, however, you are personally responsible for your account and what your insurance does not cover. Please note, co-pays are due at the time of service. Good Faith Estimate Notice

Under the No Surprises Act, patients who do not have insurance or who are not using insurance have the right to receive a Good Faith Estimate explaining the expected cost of medical care.

You have the right to receive a Good Faith Estimate for the total expected cost of non-emergency medical services, including related services or supplies.

You may request a Good Faith Estimate before scheduling a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you may dispute the bill.
