After thoughtful consideration, HorizonView Health (HVH) will be transitioning to a membership-based practice model beginning October 1st, 2025.
Our ongoing mission is to provide the highest quality care while remaining an independent medical practice. As the healthcare environment continues to change—with lower reimbursements and more uncompensated services for independent providers—this new model allows us to continue delivering the exceptional care you deserve.
Membership-based practices have grown in popularity across the region, benefiting both patients and providers by allowing for more time, flexibility, and support. This approach will help us expand the level of service we offer beyond what is typically covered by insurance, ensuring more comprehensive and personalized care.

Insurance

Your existing insurance coverage, including copays, co-insurance, and deductibles, will remain unchanged. Any services currently billed to your insurance will continue to be billed as usual.

Membership Benefits

The membership fee is separate from your insurance and provides access to enhanced services, such as:
- Extended visit times for more in-depth care
- Same-day appointments (as available)
- Secure messaging with your care team through MyChart
- Prescription refills (up to 12 months, when appropriate)
- Clinical staff phone triage for minor concerns
- 10% discount on retail products at The Spa by HorizonView

Annual Membership Fees

- Adults (ages 26+): $240/year
- Children and Young Adults (ages 0–25): $25/year

Start by reviewing the Membership Terms & Conditions. Once you’ve done that, select the appropriate payment option below to complete your enrollment. Your membership becomes active as soon as your payment is processed, and it will automatically renew each year to ensure uninterrupted access to your benefits.

Please note: Our office has a limited number of membership spots available. Once our practice reaches capacity, we will not be able to accept new patients. We encourage you to enroll early if you wish to remain under our care.
