Our ongoing mission is to provide high-quality care while remaining an independent medical practice. As the healthcare environment continues to evolve—with lower reimbursements and increasing administrative burdens for independent providers—this program helps support additional patient services, administrative conveniences, and care coordination efforts that extend beyond what is typically covered by insurance.

Membership programs have become increasingly common across the region and are designed to support enhanced patient communication, scheduling flexibility, and other non-covered program benefits. This approach allows our team to continue investing in personalized patient support and expanded service offerings.

Here’s what to expect

Insurance

Your existing insurance coverage, including copays, co-insurance, and deductibles, will remain unchanged. Any services currently billed to your insurance will continue to be billed as usual.

Membership Benefits

The membership fee is separate from your insurance and applies to non-covered services, administrative conveniences, and program benefits, which may include:

  • Extended appointment availability for more in-depth discussions and care planning
  • Expanded scheduling convenience, including same-day appointment availability when possible
  • Secure messaging access with your care team through MyChart
  • Enhanced prescription coordination and refill support, when clinically appropriate
  • Clinical support staff communication for select non-urgent concerns during business hours
  • 10% discount on regularly priced retail products at The Spa by HorizonView

Covered medical services are billed separately to insurance in accordance with applicable health plan benefits.

Annual Membership Fees

  • Adults (ages 26+): $240/year
  • Children and Young Adults (ages 0–25): $25/year

Start by reviewing the Membership Terms & Conditions. Once you’ve done that, select the appropriate payment option below to complete your enrollment. Your membership becomes active as soon as your payment is processed, and it will automatically renew each year to ensure uninterrupted access to your benefits.
