Please note: New patients may begin with a 30-day trial membership. No upfront payment is required to submit your paperwork. Membership billing will only begin if you choose to continue after the trial period.

If you are visiting our office for Weight Loss or Nutrition services only, the membership fee does not apply. All other appointment types are subject to the membership fee of $240 for adults and $25 for patients under the age of 26 after the trial period.
